IMPORTANT: This form is for veterinarians, veterinary teams, & students who are signing up for CE courses. Our clinic hours are Monday - Thursday, 7:30 AM to 4:30 PM. If you register outside these hours, a team member will contact you as soon as possible during business hours to complete your registration.
